• Two forms of ID with your signature on them, one MUST be a photo ID. Examples of acceptable ID’s include a valid Connecticut driver’s license, passport or a credit card with a signature (Social Security cards and birth certificates are not acceptable).
• Before arriving for your appointment, be sure your documents are complete and ready for signature. DO NOT SIGN THE DOCUMENT(S) UNTIL YOU APPEAR BEFORE THE NOTARY TO WITNESS.
